Good incident culture is blameless for the same reason good superhero stories are: the interesting question is never who caused it, it is what the system allowed. Missing alerts, no rollback path, one person holding all the context – those are the villains.
Write the postmortem. Fix the category, not the instance. And keep the pager honest, because responsibility without sleep is just attrition with extra steps.
